Exceptional Chiropractic Welcomes New Patients
At Exceptional Chiropractic, our aim is to remove any stress associated with coming to our Ham Lake office for the first time and to make you feel comfortable.
What can you expect on your first visit?
When you arrive you will be greeted by our front desk staff who will ask you for your driver’s license, insurance card (if you are using insurance) and payment. The driver’s license and insurance card will be scanned into our system and returned to you.
We offer online forms completed in advance of your appointment in-office through a secure portal/personal profile, or you can complete your intake forms at our office as our staff can instruct on how to use the computer tablet to enter your medical history (including surgeries), family medical history, medications/supplements and your current location(s) of pain (called “Complaints”).
The doctor will conduct the review of history, examination, performing an adjustment as needed, then will make individualized recommendations for future visits/treatment plan.
Please have the following available for your first appointment:
- Your insurance card (if using insurance)
- Your driver’s license
- A credit card for payment of any balances owed (such as exam fees, copay, co-insurance, deductibles, or self-pay services)
- A list of medications/supplements (including over the counter medications)
- Your family medical history
- Surgeries you have had
- Any imaging completed related to your complaint area(s)
